Investment 1.2 Home as the first place of care and telemedicine 1.2.1 - IHC (Integrated Home Care)

The measure aims to improve the care of people with chronic illnesses, with a focus on the over-65s. Linked to this primary objective is the complementary goal of increasing the number of patients cared for in their own homes. For the Veneto Region, the goal concerns taking care of 10.98% of the over-65 population in IHC (Integrated Home Care).

Purpose of the measure

In order to complete the implementation of telemedicine services and organizational models related to the management of chronic conditions, the project in question finances the regional activities of integration of the Regional Management Systems and the Regional Minimum Telemedicine Services  with the National Telemedicine Platform, as well as the specific integrations to be carried out for local systems.
