Investment 1.2 Home as the first place of care and telemedicine 1.2.1 - IHC (Integrated Home Care)

The measure aims to improve the care of people with chronic illnesses, with a focus on the over-65s. Linked to this primary objective is the complementary goal of increasing the number of patients cared for in their own homes. For the Veneto Region, the goal concerns taking care of 10.98% of the over-65 population in IHC (Integrated Home Care).

Purpose of the measure

The measure arises from the need to encourage the adoption of telemedicine throughout the treatment process with particular attention to chronic patients, to integrate telemedicine solutions with digital healthcare systems, in particular with the Electronic Health Record, to measure the interventions and incentivize the best ones. Telemedicine, teleassistance and telemonitoring will be structured in the network of citizen services, with a view to strengthening the territorial scope of assistance, helping to ensure equity in access to care, improve the quality of life and inclusion of patients.
