C1 Investment 1.2 Home as the first place of care and telemedicine 1.2.1 - IHC (Integrated Home Care)

The measure aims to improve the care of people with chronic illnesses, with a focus on the over-65s. Linked to this primary objective is the complementary goal of increasing the number of patients cared for in their own homes. For the Veneto Region, the goal concerns taking care of 10.98% of the over-65 population in IHC (Integrated Home Care).

Purpose of the measure

This investment aims to improve the care of people with chronic illnesses, with a focus on the over-65s. The specific goal is increasing the number of patients cared for in their own homes through IHC (Integrated Home Care). The activities will include monitoring the proportion of over-65 population cared in IHC by Local Health Unit, as compared with the goal of 10.98% for the Veneto Region, and identifying actions aiming at increasing such proportion and improving the quality and completeness of data recording within IHC databases.
